surely if the fly wheel was resurfaced, the guy would have seen any warping

I would be looking at the rear DS as well It should have a couple inches of movement before bottoming out. When the 3rd rotates up, there could be some major binding causing the bucking. Can you see any rub marks on the yoke or straps? Try removing the rear DS and run front only that may be an easy way to tell if it actually has to do with the rear DS.
Other than tha I'm not sure what could be wrong, sounds like you've covered everything else.

I did remove it and problem went away. However I'm looking at leafs now, trying to figure out if I installed them backwards which might explain the shaft not fitting right now

Springs are installed correctly. If they were installed the other way it should stretch the wheel base. Could be that they were installed the other way before. Who knows.

The lift blocks aren't helping things but something else is going on. I worked on one a few years back that bucked really bad on take off. It wouldn't do it in low range. Only high range when starting from a stop. It was the flywheel. With the price of new, I never resurface them. Could be the culprit still, not sure. What kind of clutch did you use, 3 finger style, or diaphragm style pressure plate?

A sloppy clutch linkage or bad motor mounts can cause these issues too.

One thought I am thinking is to see if it does it in 2wd low range. If it is a drive shaft issue, it might still do it??? Torque on the clutch itself would be less, but drive shaft should be seeing the same forces. My guess is it is still something with the clutch instead of the drive shaft.
